Protocol·Pain and Fatigue·Intermediate·Reviewed June 9, 2026

Fibromyalgia Support Protocol.

Adjunctive support for fibromyalgia symptom burden focused on mitochondrial energy, sleep, and correcting low vitamin D. It complements clinician-led diagnosis, graded activity, sleep care, and prescribed therapies; stop or review any supplement that worsens symptoms.

In short

The fibromyalgia support protocol in brief.

A quick summary. The full stack, with dose and timing for each supplement, is below.

The Fibromyalgia Support Protocol is an intermediate stack of 5 supplements aimed at pain and fatigue: Coenzyme Q10, Magnesium Malate, Acetyl-L-Carnitine, Vitamin D3, and Melatonin. 3 are core and the rest are optional add-ons, at roughly $35-60/mo. Each supplement below lists its dose, timing, role, and the evidence behind it.

Coenzyme Q10

Small fibromyalgia trials suggest Coenzyme Q10 may support mitochondrial function and symptom scores, but the evidence base remains limited. It is reasonable as adjunctive support, not a primary treatment.

Magnesium Malate

Magnesium malate has a small pilot-trial history in fibromyalgia and may be most useful for people with low magnesium intake, muscle tenderness, or sleep tension. Evidence remains preliminary.

Acetyl-L-Carnitine

Acetyl-L-Carnitine is studied for pain and fatigue conditions and may support mitochondrial fatty acid transport. Fibromyalgia evidence is still emerging, so use it as a time-limited trial with symptom tracking.

Vitamin D3

Vitamin D deficiency can worsen widespread pain in some people, and supplementation is most rational when levels are low. It should be framed as deficiency correction, not a fibromyalgia cure.

Melatonin

Sleep disruption amplifies fibromyalgia pain sensitivity. Melatonin may improve sleep timing and some symptom domains, but it should be used alongside broader sleep and pain care.

Why it works together

How the pieces combine.

The mechanistic rationale for stacking these together rather than taking them in isolation.

  • Coenzyme Q10 and Acetyl-L-Carnitine focus on mitochondrial support while Vitamin D3 corrects a common pain-amplifying deficiency.
  • Magnesium Malate and Melatonin are best taken later in the day to support muscle relaxation and sleep quality.
  • Use symptom tracking and clinician review, because fibromyalgia care often needs sleep, movement, pain, mood, and medication planning together.
