What is happening. Cholestyramine can bind and reduce intestinal absorption of folate, and folate deficiency has been documented with prolonged therapy, especially in children. While this primarily affects folate, taking other supplements at the same time as the resin also risks reduced absorption.
Mechanism. The anion-exchange resin physically binds folate and other negatively charged or polar molecules in the gut lumen, reducing their absorption.
Recommendation. Administer folate and B-vitamin supplements separately from cholestyramine, ideally at least 1 hour before or 4 to 6 hours after. Monitor folate status during long-term therapy, particularly in children and pregnant patients.
