What is happening. Elderberry blocks influenza H1N1 attachment in vitro and zinc lozenges (acetate or gluconate) reduce common cold duration in meta-analyses. Combined with oseltamivir, which inhibits viral neuraminidase, the mechanisms are non-overlapping. Mechanism. Elderberry flavonoids block influenza hemagglutinin attachment; ionic zinc disrupts rhinovirus binding to ICAM-1 and supports T-cell function; oseltamivir blocks neuraminidase. All three mechanisms target different stages of viral infection.

Recommendation. Elderberry zinc lozenges are a reasonable adjunct during acute influenza alongside oseltamivir. Limit to the labeled course (typically less than 2 weeks) to avoid copper depletion. Do not exceed 75-100 mg elemental zinc daily acutely.
