What is happening. Famciclovir is generally well tolerated and, unlike antibacterial agents, does not disrupt the gut microbiome, so it does not kill probiotic organisms. Probiotics may be used supportively if antiviral therapy is accompanied by mild gastrointestinal upset, with no expected loss of antiviral effect.
Mechanism. Famciclovir and its active form penciclovir are selective antiviral agents and have no meaningful antibacterial activity against probiotic species, so probiotic viability is not impaired and antiviral exposure is not altered.
Recommendation. Probiotics and famciclovir can be taken together without timed separation. There is no recognized pharmacokinetic interaction. Continue famciclovir as prescribed.
