What is happening. Ginkgo is often used for cognition but may increase bleeding risk and has seizure case reports. Rivastigmine can cause syncope and may be used in frail patients where adverse events have high consequences.
Mechanism. Potential additive neurologic risk and independent bleeding tendency in a vulnerable population.
Recommendation. Avoid unsupervised combination; review bleeding risk, falls, and seizure history with the clinician.
