What is happening. St. John's Wort is a potent inducer of CYP3A4 and P-glycoprotein and may accelerate the metabolism of ginsenosides, potentially reducing the effectiveness of American ginseng.
Mechanism. St. John's Wort hyperforin activates the pregnane X receptor, inducing CYP3A4 and P-glycoprotein, which can increase first-pass metabolism and efflux of ginsenosides and lower their systemic exposure.
Recommendation. Be aware that concurrent St. John's Wort may blunt the expected effects of American ginseng. Separating doses does not overcome enzyme induction; consider this when judging efficacy.
