What is happening. Berberine may inhibit CYP3A4 or P-glycoprotein and could increase itraconazole intolerance in susceptible patients.
Mechanism. Potential transporter and CYP3A4 inhibition could increase azole exposure; evidence is limited but the interaction direction is plausible.
Recommendation. Avoid high-dose berberine unless the prescriber has reviewed the full medication list.
