What is happening. St. John's Wort has serotonergic effects and is unsafe with irreversible MAOIs.
Mechanism. Serotonergic herb activity during MAO-A and MAO-B inhibition increases serotonin syndrome risk.
Recommendation. Do not use St. John's Wort with tranylcypromine.
