What is happening. Vortioxetine may increase bleeding tendency through serotonin effects on platelets. High-dose fish oil can have mild antiplatelet effects.

Mechanism. Serotonergic antidepressants impair platelet serotonin uptake; omega-3 fatty acids can modestly reduce platelet aggregation.

Recommendation. Use usual dietary or standard supplemental fish oil cautiously; monitor for bruising or bleeding, especially with anticoagulants, antiplatelet drugs, NSAIDs, surgery, or bleeding disorders.
