What is happening. Rabeprazole can reduce non-heme iron absorption, making oral iron response less reliable.
Mechanism. Higher gastric pH reduces iron solubilization and conversion to absorbable forms.
Recommendation. Monitor ferritin and hemoglobin if iron is prescribed; consider timing, vitamin C coadministration, or alternative iron strategies when response is poor.
