What is happening. Fenugreek fiber can slow carbohydrate absorption and may lower glucose modestly. Liraglutide also lowers glucose and delays gastric emptying, so combined use can increase gastrointestinal effects and may add to glucose lowering.
Mechanism. Fenugreek's soluble fiber and seed constituents may slow carbohydrate absorption. Liraglutide activates GLP-1 receptors, slows gastric emptying, and augments glucose-dependent insulin release.
Recommendation. Use fenugreek cautiously with liraglutide if you have diabetes or significant nausea, reflux, vomiting, constipation, or diarrhea. Tell the prescriber if insulin or a sulfonylurea is also used, and follow the diabetes glucose-monitoring plan during starts, stops, and dose changes.
