Lisinopril and NAC, a synergy.

NAC has direct ACE-inhibitory activity (about 30% reduction in ACE activity in vivo) and provides sulfhydryl groups that may amplify nitric-oxide-mediated vasodilation. Mechanism. NAC provides sulfhydryl donors that directly inhibit ACE activity and amplify nitric oxide bioavailability by reducing oxidative inactivation. These actions complement lisinopril's effects.

Recommendation. If you are considering NAC (typically 600-1200 mg/day) for other indications while on lisinopril, monitor home blood pressure for additional drops. Tell your prescriber so your lisinopril dose can be reviewed if needed.
