What is happening. Panax ginseng has antiplatelet activity, and combining it with high-dose fish oil, which also reduces platelet aggregation, may additively increase bleeding risk.
Mechanism. Ginsenosides inhibit platelet aggregation and may affect the coagulation cascade, while omega-3 fatty acids reduce thromboxane A2 production; combined antiplatelet effects can prolong bleeding time.
Recommendation. Use caution at high combined doses, especially before surgery or in those on anticoagulant or antiplatelet drugs. Discontinue ginseng at least one to two weeks before scheduled surgery.
