What is happening. Terbinafine is associated with idiosyncratic hepatotoxicity, including cholestatic hepatitis and rare cases of liver failure requiring transplant. Mechanism. Terbinafine's reactive allylic aldehyde metabolite (TBF-A) is implicated in hepatocyte injury. Alcohol induces CYP2E1 and depletes glutathione, compounding hepatotoxic stress.

Recommendation. Avoid alcohol throughout the terbinafine course and for at least a few days after it ends. Report any nausea, dark urine, abdominal pain, or yellowing of skin or eyes immediately.
